Cutting Surgical Wait Times, Province-Wide

The Challenge

Saskatchewan's provincial health system faced a growing surgical backlog: at the start of their Lean transformation, over 15,000 patients were waiting more than three months for surgery. Early improvements helped individual steps in the process, but overall wait times weren't moving, the real constraint hadn't been found yet.

The Approach

The team started by mapping the entire patient journey, then used a series of focused improvement events to find where time was actually being lost. A 3-day reminder call before surgery cut missed appointments from nearly 50% down to under 10%. From there, the team worked through surgical room turnover, sterile processing scheduling, and recovery bed availability, eventually tracing the real bottleneck all the way to medicine floor discharge delays, several steps removed from the operating room itself.

The Results

Patients waiting over 90 days for surgery dropped by 75% province-wide. In one region alone, Regina Qu'Appelle, a second wave of focused improvement drove a further 78% reduction, from 5,704 patients down to 443.

The Bigger Picture

This same discipline, following the real process instead of assuming where the problem is, is one part of a broader methodology, training teams to run continuous, Lean improvement long after we're gone.
